For Families
You're not alone in this. We're here to help your family member live a full, connected life — and to support you along the way.
Getting Started
Starting home and community-based services can feel overwhelming. Here's a simple overview of how it works:
- Referral: Anyone can make a referral — a family member, doctor, county board, or service coordinator.
- Assessment: We meet with the individual and their family to understand needs, goals, and preferences.
- Individual Service Plan (ISP): A plan is created around the person — what they want to achieve, what support they need, and how often.
- First day: We introduce the support team, set routines, and start building the relationship.
We're with you at every step. If you have questions before, during, or after — call us.
What Is HCBS?
HCBS stands for Home and Community-Based Services. It's a Medicaid program that helps individuals with developmental disabilities live at home and participate in their community instead of in institutions or larger facilities.
Through Ohio's HCBS waivers — IO, Level One, and SELF — individuals can receive services tailored to their needs, including:
- In-home personal care and homemaker services
- Transportation to appointments, work, and activities
- Remote supports and assistive technology
- Shared living arrangements
- Family and caregiver supports
Keeping Your Loved One Safe
Safety is never optional. All Strive staff complete background checks, specialized DD training, CPR/first aid certification, and ongoing professional development. Our team is trained in person-centered support, de-escalation, and emergency protocols.
Incidents are documented, reported to the appropriate county board, and followed up on with families as required by Ohio DODD guidelines.
